We will send a packet of information that contains forms that need to be completed and brought to your first visit.

Patients under 18 may need to be accompanied by an adult (a parent or legal guardian) on their first visit. Parent or legal guardian may also need to be present during the first visit. Please check with your doctor or therapist before coming to your appointment.
TO DIVORCED PARENTS OF MINOR CHILDREN: In order for us to evaluate and treat minor children, it will be necessary for you to bring a copy of the custody order with you at the time of your first visit. In addition, the parent who accompanies the child to their appointments will be responsible for paying any copays or deductibles due at the time of the visit.
What To Bring
First, come 10 minutes early. There is always first-time paperwork to fill out, and we will need to see your license and insurance card. Also remember to bring the completed papers you received in the mail/downloaded (Registration Forms, Adult Forms, and/or Child Forms). We will also need your copay (on the way in). So remember to bring those things with you when you come.
If you need to, remember to print directions to our offices.
Followup Appointment
Appropriate followup appointment will be made following the initial evaluation.
