What to bring
Please remember to bring the following to your appointment with us:
- License and insurance card. We will also need your copay or coinsurance.
- Children under 16 must be accompanied by a parent or a legal guardian.
- TO DIVORCED PARENTS OF MINOR CHILDREN: Bring a copy of the custody order at your first visit. The parent that accompanies the child to their appointments will be responsible for the copays, coinsurance or deductible payment.
